Who's Eligible?

Our service is open to those who qualify for home-based care through Medicaid, Medicare Advantage, or private insurance.

Even if past programs (like CDPAP) have changed, we offer the same hands-on assistance to help families employ trusted caregivers.

Still Interested?

Although the CDPAP name may no longer apply, our service offers the same flexibility: you choose the caregiver, we handle the backend.

It’s care designed by you, supported by a team.
